CellBank collects personal information in several ways. If you choose to enroll in the collection and storage of your tissue,, CellBank asks for certain personal information, including but not limited to your name, address, e-mail, zip code, credit card number, phone number, and government-issued identification number as well as certain health-related information such as the expected surgery date, your medical history, hospital the surgery will take place, and doctor/surgeon. Once you enroll, you are no longer anonymous to CellBank – you are given a CellBank identification number and are able to take full advantage of CellBank’s offerings.

You may request information about CellBank services and current clinical uses without enrolling. We may ask for your contact information, medical history and other information that will help us assist you. Some information is required while other information may be submitted at your option. In addition, we may ask you for personal information at other times, including, but not limited to, when you answer a survey, when you enter a sweepstakes, contest or promotion sponsored by CellBank, and when you report a problem with one of our sites or services. Who is collecting information?
